Completing Your Child’s Case History Form. A short guide to help you prepare

As part of your child’s Assessment of Need, we will ask you to complete an online Case History form.

Before you begin please be aware that the Case History form is a live online form, which means:

  • It times out after 30 minutes
  • If the form times out, anything you have entered will be lost
  • You will need to start again from the beginning
