Constellation Healthcare provides multidisciplinary paediatric assessment and intervention services in partnership with healthcare organisations across Ireland. We support Children’s Disability Network Teams (CDNTs) and other healthcare providers to expand capacity and support access to timely, high-quality care for children and young people.
Our experienced multidisciplinary team works alongside existing services to deliver flexible, clinically led support across assessment, intervention, care planning, and service delivery. We operate within established governance frameworks and collaborate closely with local teams to ensure continuity of care and positive outcomes for children and adolescets and their families.
